The following Liability Waiver and Policies must be read through and agreed to prior to Reworks Massage and Tech LLC providing any massage therapy services.

Setup Timing

Your therapist(s) will arrive 15 minutes prior to your group’s booked start time. Please have an office or open space available for them to setup in! Also, let us know if there’s building codes, stairs only or weird parking directions we need to know in advance.

Session Timing

Your therapist(s) should have provided an optional sign up sheet for your employees and staff to use. You do not have to use this sheet, but it can make getting everyone equal time on the chair(s) easier!

If you choose to use your own signup sheet or just let people show up, please let us know in advance so we can be prepared.

Please have your lucky staff/employees arrive about a minute or two before their expected time. We will work to make sure everyone gets on the chair who wants it. 🙂

Session Liability Waiver

Every client must read and agree to the following prior to receiving their massage. There will be a waiver signing sheet provided when we arrive; no need to have everyone print a copy out and sign it!

• I understand that although massage can be very therapeutic, relaxing and can reduce muscular tension, it is not a substitute for medical examination, diagnosis, care and/or treatment.
• I understand that this is a therapeutic massage and any sexual remarks or advances will terminate the session at full cost without refund.
• Being that massage should not be done under certain medical conditions, I affirm that I have answered all questions pertaining to all medical conditions truthfully and have informed Reworks Massage and Tech LLC/Cori Tate Barker, LMT of any and all medical conditions, especially those that are contraindicated (will be worsened by massage).
• I understand my responsibility to communicate:
  • Discomforts such as pressure, temperature, chair position, music, conversation.
  • Changes in medical conditions.
  • Expectations for the session and during the session.
• I hereby for myself, my heirs, administrators and assignees, waive and release any and all rights and claims of damages that may hereafter accrue to me, or which I have against Reworks Massage and Tech LLC or Cori Tate Barker, LMT for all and any injury, loss, pain and suffering, and damages suffered by me during the course of or in any way connected to any and all massage therapy sessions, past, future and present.

Rescheduling

Cancellation

Cancellations less than 24 hours in advance will forfeit the booking total cost.

No-Show

Well, if you don’t make it and don’t call or text or email then we keep your money and take a nap!
